Thank you for volunteering to drive to support athletics. Please provide proof of vaccination and assemble the following items and submit them via the PALY DRIVERS CLEARANCE GOOGLE FORM or deliver hard copies to Beth Martin in the Paly Tower Building and send an email to [email protected] confirming you have completed driver clearance.

1) Private Vehicle/Volunteer Driver Form

2) Copy of your driver's license

3) Copy of your insurance declarations page (Note: If you have State Farm, you will need to request it.)

4) Proof of Vaccination (image of card or QR code)
